จากปัญหาด้านสุขภาพของผู้สูงอายุที่เพิ่มมากขึ้นไปพร้อม ๆ กับการเพิ่มขึ้นของประชากรผู้สูงอายุ กรมอนามัย ได้ดำเนินแผนงานส่งเสริมสุขภาพผู้สูงอายุตั้งแต่ปี พ.ศ. 2555 จุดมุ่งหมายที่สำคัญคือ การส่งเสริมและปรับพฤติกรรมสุขภาพผู้สูงอายุ มีการเข้าถึงบริการสุขภาพอย่างถ้วนหน้า มีระบบบริการที่มีคุณภาพ และส่งเสริม

ปัจจุบันผู้ป่วยไตวายเรื้อรังระยะสุดท้ายมีทางเลือกในการรักษามากมาย เพื่อให้มีชีวิตที่มีคุณภาพดีและอายุยืนยาว เพียงแต่ทางเลือกเหล่านั้นยังไม่ใช่ทางที่ผู้ป่วยเลือกเป็นทางแรก การรักษาแบบประคับประคองถือเป็นหนึ่งในทางเลือกที่ทำให้ผู้ป่วยและครอบครัวมีคุณภาพชีวิตที่ดีและอายุยืนยาวในบั้นปลายของชีวิต

เสียงจากหลายภาคส่วนสะท้อนถึงการเปลี่ยนแปลงนโยบายล้างไตอย่างฉับพลันในหลายมุมมอง ทั้งเป็นการเพิ่มสิทธิประโยชน์ทางสุขภาพให้เท่าเทียมกัน เกิดศูนย์ไตเทียมเพิ่มขึ้นอย่างรวดเร็ว แต่ในทางกลับกับคุณภาพของบริการกลับถดถอย มีการรับผลประโยชน์ที่ไม่เหมาะสมเกิดขึ้นในระบบสุขภาพ ฉะนั้นหากมีการเปลี่ยนแปลงนโยบายอี

นโยบายการบำบัดทดแทนไตใหม่ 2565 ให้สิทธิผู้ป่วยไตวายได้เลือกวิธีการล้างไตวิธีไหนก็ได้โดยไม่ต้องร่วมจ่ายเชกเช่นในอดีต ซึ่งการเปลี่ยนแปลงที่เกิดขึ้นก่อให้เกิดผลกระทบทั้งทางด้านบวกและลบ ทำให้เกิดการใช้ทรัพยากรมากกว่าแต่ได้รับประโยชน์ทางสุขภาพโดยรวมต่ำกว่าเมื่อเทียบกับนโยบายเดิม
